Step 1: Log in to your Member Portal at member.coreagerx.com

Step 2: Navigate to your Dashboard

Step 3: Click the "Complete Medical Intake" button

The intake takes less than 5 minutes to complete.

What You'll Need to Provide

  • Health history — Medical conditions, current medications, allergies
  • Photo ID — A clear, readable government-issued ID
  • Full-body photos — Required for your provider's evaluation

What Happens Next

After you submit your intake:

  1. Provider Review — A licensed provider reviews your information (allow 1-3 business days)
  2. Approval or Follow-Up — Your provider will either approve your treatment or message you in the portal if they need additional information
  3. Pharmacy Processing — Once approved, your prescription is sent to the pharmacy for compounding

Important Notes

  • Check the portal regularly — If your provider needs more information, they'll message you directly in the portal, not always by email
  • Respond promptly — Delays in responding to provider requests will delay your order
  • Orders active beyond 30 days from purchase without a completed intake are considered fulfilled. No refund will be issued after this period.
