My Medication Isn't Working — What Should I Do?

Give It Time

Weight loss medications like tirzepatide and semaglutide work gradually. It's normal to not see dramatic results in the first few weeks, especially at a starting dose.

Typical Timeline

TimeframeWhat to Expect
Weeks 1–2Medication begins working; effects may be subtle
Weeks 3–4Many patients notice reduced appetite
Weeks 4–8Measurable weight loss typically begins
Months 2–3+More significant results as dose is optimized

Some patients don't notice appetite changes or weight loss until their dose is increased. This is normal and expected.

Common Reasons for Slow Progress

  • You're on a starting dose — Starting doses (like 2.5mg) are meant to help your body adjust, not necessarily to cause significant weight loss
  • It's too early — Give your current dose at least 4 weeks before evaluating
  • Diet and activity — The medication works best combined with a balanced diet and regular movement
  • Individual variation — Everyone responds differently; some people need higher doses to see effects

What To Do

1. Talk to Your Doctor

Click Talk to Doctor in the left sidebar of your member portal, select your order, and let your doctor know:

  • How long you've been on your current dose
  • Whether you've noticed any appetite changes
  • Your current weight compared to when you started
  • Any side effects

2. Complete Your Check-In

At your scheduled check-in (around week 4–5), your doctor will evaluate your progress and may recommend a dose increase.

3. Track Your Progress

Small changes add up. Track:

  • Weekly weight (same day, same time)
  • Appetite and hunger levels
  • Energy and how you feel overall

When to Be Concerned

Contact your doctor if you:

  • Are gaining weight despite following recommendations
  • Have been on an optimized dose for 8+ weeks with zero change
  • Are experiencing side effects that prevent you from taking the medication

Remember: This is a marathon, not a sprint. Most patients see their best results over 3–6 months as their dose is gradually optimized.
